What's Important to You?

That's where every conversation starts. For more than 50 years, Southwest Michigan families have trusted Region IV Area Agency on Aging with the questions aging brings.

Explore What's Right for You

It’s your life, and where you receive support should be your choice. We’ll listen before we present options. We’ll offer an unbiased view of the supports and resources available. And we’ll talk alongside you as you decide what fits your life.

It starts with one call to our Info-Line for Aging & Disability at 1.800.654.2810. An Information & Access Specialist will help you find what you’re looking for and often uncover support you didn’t know to ask about.

Southwest Michigan Community Care Hub

When health care systems and community providers kept missing each other in ways that left real people stuck or stranded, together we built the Southwest Michigan Community Care Hub. Today, the Hub connects health plans, providers, and community organizations through one coordinated system, with Region IV at the center. It’s the first of its kind in Michigan and one of four hubs leading this work nationally.
